The mesh should be used in the form in which it is provided without being cut. 

Parietex™ optimized composite mesh should be hydrated in its original blister before being handled. This is carried out by immersing it completely in a sterile saline solution for several seconds to ensure its conformability and flexibility.

Yes, sutures are necessary.  The technique used to anchor the mesh (suture and/or tack) is left up to the surgeon.
