Help us keep your patient record up to date. Patient ID cards are a free service and it will only take 3-5 minutes to fill out the online form.

Device ID Card

Why Have an ID Card?

We recommend that all Medtronic patients with an implanted device carry an ID card in order to identify your device information in case of emergency.

The ID card includes a toll-free number to contact Medtronic, and helps maintain current and accurate information for your record.

How Do We Use the Information You Provide?

Medtronic places very high value on safety and security of your private information. The information you provide will be transmitted over a secure network, and is used to keep your device record current and accurate. We will use the information provided to supply you with an updated ID card, which will be mailed to you shortly after completing our online form.

All of the information you provide is used in compliance with FDA requirements and as otherwise described in our Terms of Use and Privacy Statement.

Who Should Have an ID Card?

Medtronic patients with any of the following devices should have an ID card:

  • CRHF device (pacemaker, defibrillator, and/or leads)
  • Neuromodulation device (drug pump and/or neurostimulators)
  • Endovascular stent
  • Heart valve, ring, and/or band

If you meet ALL of the following criteria, please use this form to update your information, and a new ID card will be mailed to the address you provide:

  • Medtronic is the manufacturer of your device and
  • Your medical device was implanted in the United States and
  • One of the following pieces of information need to be updated:
    • Name
    • Address
    • Phone Number
    • Following Physician
    • Social Security Number

ARE YOU IMPLANTED WITH AN INSERTABLE CARDIAC MONITOR (ICM)?

Medtronic patients with insertable cardiac monitor implants (e.g., Reveal LINQ ICM, LINQ II ICM, etc.) do not require patient ID cards. However, if you would like an ID card, you can download one below and print at your convenience.
