STANDARD AND SUPPLEMENTAL WARRANTY Mechanical Circulatory Support

OVERVIEW

Medtronic is committed to assisting hospitals with managing the complexities of product warranties. The warranty information on this page is specific to Canada’s Mechanical Circulatory Support (MCS) product line, which includes the HeartWare™ HVAD™ System.

 

MCS WARRANTY PROCESS

Medtronic Canada has a defined standard warranty process for MCS products suspected to be functioning outside of normal tolerances. To initiate a warranty request, the following must occur within 30 days of the product malfunction date:

  • Complete, acknowledge, and email a Medtronic Standard and Supplemental Limited Warranty Claim Form to the Medtronic Warranty Team at  canada.crdmbsr@medtronic.com within 30 days of the product malfunction date.
  • Return the product to Medtronic Canada within 30 days of the product malfunction date to 95 Market Drive, Milton, Ontario  L9T 4Z7. Refer to the MCS Warranty Cards below for Return Instructions.

MCS Warranty Claim Form

For timely processing, you will need to complete the PDF form electronically and provide an e-signature. Within 30 days of the product malfunction date, email the warranty claim form to the Medtronic Warranty Team at canada.crdmbsr@medtronic.com. You may download and save the warranty form for future use and/or save the completed warranty form for your records.

 

MCS WARRANTY CARDS

HeartWare HVAD System Limited Warranty Card: This card provides Medtronic’s Warranty Terms and Conditions for HeartWare HVAD System products.

HeartWare Monitor Kit Limited Warranty Card: This card provides Medtronic’s Warranty Terms and Conditions for the HeartWare Monitor Kit.
